Clinical Perspective
Ultrasound and fluoroscopy are complementary rather than competing technologies. The best imaging method is the one that provides the clearest and safest pathway to the intended target for that individual patient.
Interventional Treatment Options
A trochanteric bursa procedure is not limited to one medication or one technique. Treatment is selected according to whether the primary goal is diagnostic clarification, rapid reduction of inflammation, biological treatment of chronic tendon pathology, removal of degenerative tendon tissue, or management of surrounding myofascial pain.
Diagnostic Local Anesthetic Injection
A small amount of local anesthetic may be used when temporary numbing of the target area would help determine how much of the patient’s pain originates from the trochanteric bursa or immediately surrounding tissues. The response is interpreted together with the examination and imaging findings rather than used in isolation.
Corticosteroid Injection
A corticosteroid injection may be selected when active inflammation is believed to be an important pain generator. The medication is delivered under ultrasound or fluoroscopic guidance to reduce inflammation and improve comfort while the patient continues rehabilitation and activity modification.
Corticosteroid treatment may provide relatively rapid relief, but it does not repair chronic tendon degeneration. Repeated steroid exposure around compromised tendon tissue should therefore be considered carefully and individualized.

Because dry needling addresses muscular trigger points rather than bursal inflammation or tendon degeneration, it is generally used as an adjunct rather than the primary intervention.
When Is TenJet® Considered?
TenJet® is not a treatment for isolated inflammation of the trochanteric bursa. It is an ultrasound-guided percutaneous tendon procedure that may be considered when persistent lateral hip pain is associated with chronic degeneration of the gluteus medius or gluteus minimus tendon and appropriate non-surgical treatment has not provided adequate improvement.
During the procedure, the abnormal tendon region is identified with ultrasound. Through a small skin opening, the TenJet device uses a controlled stream of fluid and suction to remove selected degenerative tendon tissue while preserving healthier portions of the tendon.
TenJet may be considered when:
- Symptoms have persisted despite rehabilitation and activity modification.
- Ultrasound or MRI demonstrates chronic gluteal tendinosis or a selected partial tendon abnormality.
- Prior corticosteroid injection provided only temporary or incomplete improvement.

TenJet cannot reconnect a complete tendon rupture, correct major retraction, or restore severe muscle atrophy. Patients with advanced tendon failure may require orthopedic evaluation instead.
Clinical Perspective
TenJet treats selected degenerative tendon tissue rather than simply suppressing inflammation around the hip. It is therefore reserved for patients whose persistent symptoms are substantially related to chronic gluteal tendon disease, not for every patient referred for a trochanteric bursa injection.
Other and Emerging Treatment Options
The treatment of persistent lateral hip pain continues to evolve. Some therapies may complement image-guided injections or provide alternatives for carefully selected patients, but they are not equally appropriate for every diagnosis.
Percutaneous Tendon Fenestration
Ultrasound-guided tendon fenestration, sometimes called needle tenotomy, uses repeated controlled needle passes through a degenerative tendon region to stimulate a healing response. It may be considered for selected gluteal tendon abnormalities but is used less frequently in our practice than other available interventions.
Extracorporeal Shockwave Therapy
Extracorporeal shockwave therapy delivers externally generated acoustic energy to the painful tendon region without placing a needle through the skin. It may help selected patients with chronic greater trochanteric pain, particularly when combined with rehabilitation. Research is encouraging, but treatment protocols and outcomes vary among studies.
Surgical Repair
Orthopedic evaluation may become appropriate when imaging demonstrates a substantial gluteal tendon tear, significant weakness, tendon retraction, progressive muscle atrophy, or persistent functional limitation despite comprehensive non-surgical treatment. Surgery is directed toward the structural tendon problem rather than the bursa alone.

What Happens During a Trochanteric Bursa Procedure?

Preparation and Positioning
The patient’s medications, allergies, medical history, prior imaging, and treatment goals are reviewed. The patient is positioned to provide safe access to the outside of the hip, commonly lying on the unaffected side or in another position selected for the imaging technique.
