BVN Ablation in Post-Fusion Spine: A Novel Posterolateral Access Technique

What do you do when a patient needs basivertebral nerve ablation — but prior fusion hardware occupies the only described access corridor?
This case report describes a novel posterolateral vertebral wall approach for four-level BVN ablation (L3, L4, L5, S1) in a patient with prior L4-5 instrumented fusion with bilateral pedicle screws and interbody cage. The published Intracept trials excluded post-fusion patients entirely. This technique — entering the lower third of the vertebral body via the posterolateral wall at approximately the 5 o’clock position — bypasses the occupied pedicles while respecting the segmental lumbar artery at mid-vertebral height.
